Referrals> New/Open/Closed/Ineligible>Select

 

In the Referrals screen you can identify Referrals and access Referral details.

 

1.     You can click the ‘New’ link on the left navigation bar. A table with New Referrals will be displayed.

2.     You can click the Select button for a New Referral and a screen with the New Referral details will be displayed.

a.     You can click on the Name link to display referral information for that patient.

                                          i.    You can click the Select Referral button to display referral information for that patient.

                                         ii.    You can click the Select Contact Entries button to display contact information for that patient.

                                        iii.    You can click any Select button to display information about the selected Stages for that patient.

                                        iv.    You can click the Cancel button to return to the Follow Up information screen.

b.     You can edit or add the following information:

For Veterans Affairs Medical Center:

·         Facility Name (choose a name from the dropdown box by clicking the down arrow)

·         Address 1

·         Address 2

·         Address 3

·         City

·         State

·         Zip/Postal

·         Country

·         Contact Name

·         Contact Email

·         Contact Phone

·         Contact FAX

 

For Primary Care Physician:

·         Name

·         Phone

·         Email

·         FAX Number

·         Address 1

c.     You can Add or Edit the following Health Factors:

·         Fragments (Note:  You can add additional information if Embedded Fragments Present is checked)

·         Injury Type

·         Blast/Explosion Type (Note: you can add additional information if Other is checked)

·         Fragments in Body

·         Radiography

·         Vehicle

·         Surgery

·         Laboratory

·         Unable to Screen (Note: you can add information if Acute Illness or Severe Cognitive Impairment is checked).

d.     You can click the Accept radio button to accept the referral for triage.

e.     You can check the For Biomonitoring or For Fragment Analysis to specify the triage.

f.      Or you can click the Accepted – No Action Required radio button.

g.     Or you can click the Ineligible radio button (Note: if you click the Ineligible radio button, an Ineligibility Reason is required for saving).

h.     You can click the Save button and any changes you made will be saved.

i.      You can click the Back button and you will be returned to the Open screen.

j.      You can click the Add Contact button and the New Log screen will be displayed.

You can add or edit the following information:

·         Person Contacted First Name (Note: This field is required for saving the information)

·         Person Contacted Last Name (Note: This field is required for saving the information)

·         Date Contact was Made (mm/dd/yyyy) (Note: This field is required for saving the information)

·         Time (hh:mm)

·         Method of contact (choose one from the dropdown box)

·         Person Contacted Telephone Number

·         Person Contacted Email

·         Person Contacted Address 1

·         Person Contacted Address 2

·         Person Contacted Address 3

·         Person Contacted City

·         Person Contacted State

·         Person Contacted Zip Code + 4

k.     You can click the Save button to save the information.

l.      You can click the Cancel button to remove the information you entered or edited and you will be returned to the New Referrals details screen.

 

3.     You can click the ‘Open’ link on the left navigation bar. A table with Open Referrals will be displayed.

4.     You can click the Select button for an Open Referral and a screen with the Open Referral details will be displayed.

a.     You can click the Name link to display referral information for that patient.

                                          i.    You can click the Select Referral button to display referral information for that patient.

                                         ii.    You can click the Select Contact Entries button to display contact information for that patient.

                                        iii.    You can click any Select buttons to display information about the selected Stages for that patient.

                                        iv.    You can click the Cancel button to return to the Follow Up information screen.

5.     You can click the ‘Closed’ link on the left navigation bar. A table with Closed Referrals will be displayed.

6.     You can click the Select button for a Closed Referral and a screen with the Closed Referral details will be displayed.

a.     You can click on the Name link to display referral information for that patient.

                                          i.    You can click the Select Referral button to display referral information for that patient.

                                         ii.    You can click the Select Contact Entries button to display contact information for that patient.

                                        iii.    You can click any Select buttons to display information about the selected Stages for that patient.

                                        iv.    You can click the Cancel button to return to the Follow Up information screen.

7.     You can click the ‘Ineligible’ link on the left navigation bar. A table with Ineligible Referrals will be displayed.

8.     You can click the Select button for an Ineligible Referral and a screen with the Ineligible Referral details will be displayed.

a.     You can click the Name link to display referral information for that patient.

                                          i.    You can click the Select Referral button to display referral information for that patient.

                                         ii.    You can click the Select Contact Entries button to display contact information for that patient.

                                        iii.    You can click any Select buttons to display information about the selected Stages for that patient.

                                        iv.    You can click the Cancel button to return to the Follow Up information screen.

d.     You can click the Clear Triage button to clear the ineligible status.  The Triage Referral section containing Accept, Accepted – No Action Required and Ineligible radio buttons will display.

g.     Or you can click the Accepted – No Action Required radio button
